For this reason, it is our practice to administer loop diuretics to patients at risk for myoglobinuric renal failure. Hemoglobinuria can also result in ARF. The pathophysiologic mechanisms are similar to those involved in myoglobinuric ARF. Hemoglobinuric renal failure is relatively rare. Hemoglobin, with a molecular weight almost four times that of myoglobin, is less readily filtered. Furthermore, when hemoglobin is released into the plasma, it binds to haptoglobin, forming a bulky, nonfilterable molecular complex. Only when the haptoglobin binding capacity is saturated (at hemoglobin concentrations greater than 100 mg per dL plasma) does hemoglobin appear in the tubular fluid.
